First Appointment
On your first appointment you'll fill out an information sheet and permission form which will include full name and address, medical history and medications, so please bring along your latest prescription copy if you won't remember or know this information on your appointment day. We'll discuss your problem together, as well as carry out a full assessment of your condition. You'll be treated as your assessment indicates and a routine appointment will be made as required.
Routine Appointments
On your routine appointment, you'll be treated for your condition. If you have any further problems they'll also be assessed and where possible treated within your appointment time. If you have any change in medication or personal details before your next appointment, please let us know as soon as possible. Please note that although all appointments are allocated 30 minutes, this is not how long your appointment will necessarily take. If you require a follow-up appointment it will be charged at the rates stated below.
